News from the Nurse-end-of-year medicine pick-up

EIGHTH GRADE PARENTS MEDICATION PICK-UP:  If your child has any medication in the nurse’s office, please plan on having an ADULT pick up the medicine during the last week of school.  Asthma inhalers can be brought home by the student. Any medication remaining after the last day of school will be discarded. 

 

SIXTH AND SEVENTH GRADE PARENTS MEDICATION PICK-UP:  If your child has any medication in the nurse’s office, I would be happy to store it in the nurse’s office for next year’s use.  A medication authorization form signed by your child’s physician is still required for the following school year.  If you prefer to pick up the medication, please do so by the last day of school.

 

CURRENT SIXTH GRADE PARENTS:  The State of Connecticut requires all sixth grade students to have a physical examination during the sixth grade school year.  An exam done during fifth grade is acceptable.  The State of Connecticut has also updated its immunization requirements for entry into 7th grade.  All children entering 7th grade must have 2 varicella vaccines (chicken pox), a Tdap booster (tetanus, diphtheria, and pertussis) and a menactra vaccine (meningitis). Your child’s completed exam and immunization update needs to be in the nurse’s office by June 30, 2016 in order for your child to be admitted into seventh grade.  Reminder notices were sent home to the parents of students who are not in compliance.  If you have an appointment over the summer, contact the nurse to inform.  Please be advised, your child will not be allowed to start seventh grade until proof of required immunizations and /or completed physical examination form is presented to the nurse’s office.

 

NOTE:  IF YOUR CHILD’S HEALTH RECORD IS INCOMPLETE, YOU WOULD HAVE RECEIVED A WRITTEN NOTICE FROM THE NURSE.
